<p>The Medical Banking Project (now part of HIMSS) studied this question for quite awhile. I blogged a series of topics awhile back ( http://www.mhimss.org/blog/author/5646 ; http://edodds.blogs.com/mblog/ ; http://blog.conmergence.com/work/ ) and the HIMSS World Bank Task Force did a recent whitepaper (January 2013 &#8211; The Healthcare Payments Hub: A New Paradigm for Funds and Data Transfers in Healthcare &#8211; http://www.himss.org/ResourceLibrary/ResourceDetail.aspx?ItemNumber=16607 ). John Casillas could add a lot but the things that stand out to me are: 1) Insurers have to be willing to reimburse for eHealth, telehealth, video consults, etc. As CMS and other fedgov depts have taken steps in this direction private insurers will follow. 2) Paper needs to be taken out of the administrative process (standards for matching 835 and 837s, etc.); last stat I heard was about 94% of paper claims had to be followed up with a phone call (not sure how current that stat is). 3) In TN, for historical legal reasons, nearly all state funds have gone to folks with an intellectual disability leaving very few resources for those who have a developmental disability. Gov Haslam and the legislature have turned that around lately but a lot more needs to be done unless we want it to remain an &#8220;open secret&#8221; that if you acquire a disability, we&#8217;ll do our dead level best to dump you on the other side of any state line. My wife Deana can get anyone more details &#8212; see http://ucpmidtn.org 4) The recent TN Broadband Summit spotlighted the reality the folks who can benefit most from technology (I&#8217;m thinking health over IP here) tend to be in those places where incumbent broadband providers can least afford to build out. The FCC e-rate program could theoretically help but right now it isn&#8217;t structured optimally. We&#8217;ll have to see what happens after Genachowski and McDowell are replaced. 5) The possibilities for economic development along these are stifled by tax structure (in just abut every state) because unless a project (which might hire many knowledge workers, in this case, healthcare related) actually causes someone to build or buy real estate (the way we capture revenue of the type that shows up in local econ dev employees metrics &#8212; and hence the way they keep their job, earn a bonus) it doesn&#8217;t count as &#8220;real revenue&#8221;. So you can have a situation like the Gig City where everyone could be using a software like BOINC or CPUsage for distributed computing on a superfast grid to build Glenn Ricart&#8217;s (US-Ignite) neighborhood supercomputer for medical research to cure cancer (say sponsored St. Judes) but under the current tax scheme that&#8217;s not a &#8220;real business&#8221;. Does any of this make sense to anyone else? 6) Obviously, I haven&#8217;t even touched mPayments coupled with mHealth. I realize I hop around here a bit from topic to topic &#8212; but maybe it will get a discussion a bit further down the path. </p>

The World Bank has become a leader in Open Data both through its own experience and work with some of the world&#8217;s pioneering Open Data initiatives. Governments at all levels are starting to see Open Data as a powerful tool for transformation of their countries into dynamic, innovative economies, with modern, citizen-centered public sectors and vibrant ICT markets.</p>
<p>The Development Data Group (DECDG), the ICT Sector Unit (TWICT) and the World Bank Institute (WBI) have partnered to help client countries use Open Data for development. As part of this global initiative, DECDG and TWICT have been awarded a grant from the Trust Fund for Statistical Capacity Building to support the implementation of Open Data initiatives in client countries.  Under this initiative, we are able to support a limited number of Open Data Readiness Assessments to diagnose what a government need to do to make its data truly open and to use open data to improve public services and government performance and transparency, as well as to create jobs and catalyze innovation throughout an economy.</p>
<p>The World Bank&#8217;s Open Government Data Working Group has recently developed an &#8216;Open Data Readiness Assessment&#8217; (ODRA) tool for conducting an action-oriented assessment of the readiness of a government &#8211; or even an individual agency &#8211; to evaluate, design and implement an Open Data initiative. The tool is added to the &#8216;Open Data Toolkit&#8217; published at data.worldbank.org/ogd. It is made freely available for others to adapt and use. We invited Andrew Stott and Jeff Kaplan, who are the main authors of the tool, to provide a briefing on the methodology to those interested to make use of the tool and share the early findings from the ongoing assessment to Ulyanovsk Oblast of Russia, the first such assessment using this methodology.</p>

The approach of Boite à innovation is an interesting example of content creation adapted to African languages and cultures and aimed at enhancing confidence and creating a digital culture for all.</p>

<p>The Office of the National Coordinator for Health IT (ONC) is seeking your participation in the S&amp;I Framework Initiative, <a href="http://wiki.siframework.org/Longitudinal+Coordination+of+Care"> Longitudinal Coordination of Care (LCC) Workgroup (WG</a>). The LCC WG is a community-led initiative charged with supporting and advancing interoperable electronic health record (EHR) systems and patient-centric interoperable health information exchange across the Long-term and Post-Acute Care (LTPAC) spectrum. Over the past year, the LCC WG has been engaged in developing a standardized patient assessment tool and refining standards and implementation guidance for consideration in Meaningful Use Stage 3. More specifically, the LCC WG and its three sub-workgroups are addressing the following challenges:</p>
<ol>
<li>Absence of an interoperable, longitudinal care plan which aligns, supports and informs care delivery regardless of setting or service provider</li>
<li>Lack of clear and harmonized terminologies and definitions for care plans</li>
<li>Lack of robust, interoperable EHR systems across the LTPAC spectrum with the ability to electronically exchange clinical information</li>
<li>Underserved community with multiple care settings and needs that exceed exchange of clinical information</li>
</ol>
<p>Key accomplishments (with hyperlinks) of the LCC WG include:</p>
<p><a href="http://sibrowser.siframework.org/siclient/view?type=artifact&amp;id=6157ccec-7d72-4f2b-b933-c5575cc8f33a&amp;name=SIFramework_LCC_UC.docx">LCC Use Case</a>. Outlines three scenarios for health information exchange between:</p>
<ol>
<li>an acute care hospital and home health agency (HHA)</li>
<li>a skilled nursing facility (SNF) and the Emergency Department (ED)</li>
<li>a Physician and a HHA. Two of the scenarios center on the Home Health Plan of Care (HH-POC), based on the CMS 485 form.</li>
</ol>
<p><a href="http://wiki.siframework.org/file/view/IMPACT%20Transfer%20of%20Care%20Dataset%2010-20-12%20-%20For%20Distribution.xls/375236566/IMPACT%20Transfer%20of%20Care%20Dataset%2010-20-12%20-%20For%20Distribution.xls">Transitions of Care and HH-POC Data Set</a>. Builds from the LCC Use Case and identifies over 480 data elements needed by receiving clinicians to safely and appropriately care for patients at times of transitions of care.</p>
<p><a href="http://wiki.siframework.org/file/view/Care%20Plan%20Glossary_v24%20CLEAN.doc/391060546/Care%20Plan%20Glossary_v24%20CLEAN.doc">Care Plan Glossary</a>. Describes a key list of care plan terms, definitions and components to be considered<br />
for MU stage 3.</p>
<p>The LCC WG is a community-led initiative that is mainly funded and driven by public and private sector partners, each of which is truly committed to overcoming the interoperability challenges in long-term and post-acute care transitions. These dedicated groups include my company, <a href="http://www.cmsociety.org/">Care Management Professionals</a>.  We hope it could also include the Case Management Society of America (CMSA) and your own firm, Case Management Innovators.</p>
<p>Given the current timeline to respond to the Health IT Policy Committee (HITPC) <a href="http://www.healthit.gov/buzz-blog/meaningful-use/set-stage-meaningful-stage-3/">Stage 3 Meaningful Use Request for Comment (RFC)</a>, the LCC WG has hosted a series of webinars to give key stakeholder groups additional information on the current recommendations and questions related to care plans and other key LTPAC priority areas including transitions of care and advance directives. The LCC WG is focused on gathering and consolidating LTPAC community comments and feedback on the existing recommendations in order to develop a unified and targeted response to influence Stage 3 MU recommendations.</p>
<p>We encourage you to review one of the four presentations and recordings of these webinars which are now available on the LCC wiki page <a href="#S&amp;I%20LCC%20WG%20MU%20Stage%203%20Request%20for%20CommentsWebinars">here</a>. The LCC WG wants to ensure all LTPAC stakeholder groups have the opportunity to shape Stage 3 MU recommendations and ultimately influence the development of a collaborative model of care that encompasses a longitudinal and comprehensive care plan.</p>

<p>Colleagues are good things. Jim Long and the programmers at GM Spring Hill Manufacturing (Compuware, EDS, GM, Xerox before my time) taught me how to divide a software problem up into modules (functional concepts in classic VB); Mike Mancour and Lonnie Harriford how the same approach can be applied well to team programming. Greg Wernke taught me to QA with diligence. John Casillas pointed me in the open source direction with a comment while working with the late John Hardin on the COMBAT initiative. Mark Charleton crystallized the business case for adopting frameworks. Freddie O&#8217;Connell mentioned he thought of me when he saw an API story, a theme which continues to resonate. David Siegel, John Hagel, John Seely Brown, Kingsley Idehen have enlightened me re: the Pull approach. There are so many more of you (too numerous to mention) which make each day land in the hope side of the ledger vs. despair. Merry Christmas, Happy Holidays and thanks for the gifts you&#8217;ve already given me.</p>

His applications have rivalled some of the world&#8217;s most popular platforms in downloads. (SOUNDBITE) (English) ABDU SEKALALA, DEVELOPER SAYING: &#8220;We have word book which is a dictionary and the Tutu translate which is basically a translator and then there is world sports which is a sports application for soccer fans.&#8221; He&#8217;s developed nine internationally recognised Apps. Wordbook is among the most successful. It earns him 1.25 dollars everytime it&#8217;s downloaded. So far that&#8217;s over 300,000 times &#8211; making him some 375,000 U.S. dollars. Africa is the world&#8217;s fastest growing telecoms market and Abdu got his big break last year after taking a Nokia training course. The mobile phone giant now sells Abdu&#8217;s Apps in its specialist Ovi store. Agatha Gikunda is Nokia&#8217;s Head of Apps in East Africa. (SOUNDBITE) (English) AGATHA GIKUNDA, NOKIA HEAD OF ECOSYSTEM SERVICES AND APPLICATIONS, EAST AFRICA SAYING: &#8220;Africa is definitely the next frontier. Developers here have a huge opportunity that they have never ever had before to create businesses that require very little start up capital. They are already trained in development so we then take them to the next step to train them in the development for a mobile phone and all you need is your computer and your idea, you publish your application for free, you select which countries around the world and that is it.&#8221; Abdu is now mentoring others and says his greatest achievement is a free App called &#8216;Uganda Theme&#8217;.
